The American Medical Association (AMA) has announced new CPT codes (90482–90484) for immunization counseling — offering pediatric providers a new way to capture reimbursement for valuable time spent counseling patients and families about vaccines, even when immunizations are deferred, declined, or the patient is underimmunized.
These codes, detailed in the March 2026 CPT® Assistant (Issue 3, page 21), recognize the increasing clinical and administrative effort required for informed vaccine discussions and education. They can be reported when no vaccine is administered or when counseling occurs outside of an immunization visit, supporting both patient engagement and practice sustainability.
Importantly, these codes are time based and selected according to the total time spent by the physician or other qualified health care professional providing immunization counseling during the encounter. Time thresholds are defined as follows:
- CPT® 90482 – Immunization counseling provided for 3 minutes up to and including 10 minutes
- CPT® 90483 – Immunization counseling provided for greater than 10 minutes up to and including 20 minutes
- CPT® 90484 – Immunization counseling provided for greater than 20 minutes
Accurate time documentation is essential to support the level of service reported and should reflect active counseling and education related to vaccine risks, benefits, and clinical recommendations.
